Chemical Approval Must be completed and reviewed prior to purchasing any new chemicals  *Does not apply to laboratory chemicals

Hepatitis B Vaccination Program  For employees participating in the University Exposure Control Plan. Please complete and return to EHS office.  
Hot Work Permit Required for welding
Needle Stick/Bodily Fluid Exposure  If you come in contact with blood or other bodily fluids, IMMEDIATELY wash with soap and warm water. Contact your supervisor & the EHS office at 336.758.7189
Spill/Incident Report For any oil/chemical spill or release on campus.
Ergonomic Assessment To request an ergonomic assessment of your work area, contact Scott Frazier at 336.758.4329 or frazie@wfu.edu or Bridget Marrs at 336.758.7189 or marrsbs@wfu.edu. In addition, please download and submit an ErgoProfile Employee Survey to the EHS office via campus mail or email to wfuehs@wfu.edu
Space Hazard Assessment Please download the appropriate Space Hazard Assessment for your area; Athletics, Biology, Chemistry, Facilities & Campus Services, Health & Exercise Science, Physics, or Scales
Space Heater Authorization Please submit a Space Heater Request and Approval form. For more information, please refer to the Portable Space Heater Requirements.
Vehicle Accident To be completed and returned to the EHS office in the event of a vehicle accident involving Wake Forest University owned property.

 

Procedures

Procedure Description 
Asbestos Management Wake Forest University is responsible for ensuring that any work that might disturb asbestos, and work with ACMs, is safely managed. The Office of Environmental Health & Safety is responsible for administering the University’s asbestos management and educational awareness program, and for monitoring compliance with State and Federal asbestos regulations.
Blood Borne Pathogens Exposure Control Plan The Exposure Control Plan is designed to provide regulatory compliance and a means by which university employees will be informed about and protected from exposures to blood and other potentially infectious materials.

Permit-Required Confined Space A comprehensive Confined Space Entry Procedure has been developed to protect Wake Forest University employees and provide guidance to contractors who may be required to enter confined spaces during the course of their work.

Hazardous Communication This comprehensive Hazard Communication Procedure has been developed to protect employees from the dangers associated with chemical substances used during the course of their work.
Hearing Conservation  Wake Forest University has implemented a Hearing Conservation Program to ensure that all employees who are exposed to hazardous noise levels are adequately  protected to prevent hearing loss.
Ladder Safety The Ladder Procedure is applicable to all persons who are employed by Wake Forest University and may use any ladder during the course of work (including portable ladders, extension ladders and fixed or permanent ladders) and had been developed to assist all university personnel with identification and mitigation of hazards and to promote a safe, efficient work place environment.
Machine Guarding Wake Forest University machine operators are responsible for operating the machines with all safeguards in place. Supervisors must ensure that safeguards cannot be defeated and that all machines are operated per the manufacturer’s specification.
Personal Protective Equipment Personal protective equipment, or PPE, is designed to protect us from serious workplace injuries or illnesses resulting from contact with chemical, radiological, physical, electrical, mechanical, or other workplace hazards.
Power Industrialized Equipment/Vehicles Industrial trucks include fork trucks, tractors, platform lift trucks, motorized hand trucks, and other specialized industrial trucks powered by electric motors or internal combustion engines.
Lockout/Tagout The Wake Forest University Control of Hazardous Energy Procedure establishes the minimum requirements for the lockout and tagout of energy-isolating devices to prevent an unwanted release of hazardous energy during any servicing, maintenance, or modification activity.Please contact the Office of Environmental Health & Safety at wfuehs@wfu.edu for questions related to Lockout/Tagout.
Respiratory Protection Based on potentials for respiratory hazards, this comprehensive standard was developed to protect Wake Forest University employees who may be required to wear respirators during the course of their work.
